254 AN AUDIT OF DISCHARGE SUMMARY QUALITY AND INTRODUCTION OF A DISCHARGE SUMMARY TEMPLATE IN AN ORTHOGERIATRIC SERVICE

Abstract Background The discharge summary is the main method of communication integrating care older adults from acute hospital admission to their destination. HIQA published standards ensure high quality and safe information transfer between settings. We designed an audit evaluate summaries orthogeriatric service before after introduction a template for use by Non-Consultant Hospital Doctors (NCHDs) on Methods Data were collected anonymously using excel data collection tool with 51 points covering seven key areas outlined in standards.
